You walk into the clinic room, your lab coat sleeves rolled up, your stethoscope askew. You're tired, and it's been a long clinic day. You have the imperiousness of someone who just finished a decade of training, and you want this “urgent” new patient add-on who “has connections” to know that you'e being put upon. However, one look at the patient as he hunches over a basin trying not to heave, and you know that this is the real deal.
As you sit down across from him, you pause for a second to look at his wife, who is gently caressing his back in slowly widening circles. Her gaze meets yours, and there is a quiet, desperate pleading in her eyes. You have seen that look before, and, somehow, it never ends well.
