A group of nine patients with a distinctive diffuse lesion of the glomerular basement membrane and the idiopathic nephrotic syndrome have been studied for a decade. These cases have been culled from a series of 500 patients with renal problems including lipoid nephrosis (idiopathic nephrotic syndrome of childhood (INSC)) and systemic diseases.
A pattern of ultrastructural alterations can be identified from the comparison of renal biopsies taken serially throughout the course of illness. Early glomerular changes are marked by the diffuse basement membrane widening, conspicuous variations in electron density and the gradual distortion of the adjacent cellular components.
