Plasma conjugated and unconjugated bilirubin were both believed to be completely protein-bound and hence unavailable for renal glomerular filtration. Our studies in dogs with ligated common bile ducts, however, suggested that glomerular filtration was the predominant mechanism of renal excretion of conjugated bilirubin. The binding of bilirubin to plasma proteins was therefore reexamined. Unconjugated bilirubin was not dialyzable from plasma. Dialysis of conjugated hyperbilirubinemic plasma did yield a small fraction of dialyzable bilirubin, sufficient to account for its renal excretion by glomerular filtration.
