The presence of free air within the connective tissue supporting structures of the mediastinum, an entity variously termed pneumomediastinum and mediastinal emphysema, has long been recognized but only recently noted as more than a rarity. The purpose of this report is to add 28 more cases to the 322 previously documented cases of pneumomediastinum. These 28 cases have been collected over a 10-year period and represent an admission incidence of 1 in 7,115. Of the 28 cases reported, there were 3 infants, 2 children, and 23 adults.
