A commercially available nonprotein polysaccharide-nucleic acid complex prepared from Pseudomonas (Piromen®) is used as a standardized stimulus to evaluate the function of the hypothalamus-pituitary-adrenal cortical axis in hospitalized or ambulatory patients. The test is performed as follows: 2.5 ml plasma are obtained from blood collected in a heparinized vacuum tube at 8 AM after an overnight fast; Piromen, 0.1 µg/kg of body weight, is given intravenously; and 2.5 ml of plasma are obtained 2 hr later. Plasma cortisol concentration is determined by a fluorometric procedure. Simplicity and lack of serious side effects recommend the test for a variety of studies.
