The mechanisms responsible for the accumulation and clearing of the alveolar material in alveolar proteinosis have not been systematically studied, and the nature of this material is still a subject of speculation.
This paper presents a synthesis of multidisciplinary studies undertaken, with the collaboration of many investigators, to elucidate these problems. The studies were performed on nine cases of alveolar proteinosis observed clinically over a period of 2 to 5 years. Pulmonary biopsies were performed in six cases, 1 to 8 weeks after bronchopulmonary irrigation, while resolution of the pulmonary infiltrates was occurring.
