Ten episodes of thrombocytopenia in five chronic alcoholics with delirium tremens are described. Platelet counts rapidly returned to normal after withdrawal of alcohol with a subsequent thrombocytosis. None of the patients had cirrhosis, splenomegaly, or evidence of folic acid deficiency. Thrombocytopenia recurred in three patients 4 to 8 weeks after discharge from hospital and resumption of ethanol intake. It is postulated that the thrombocytopenia was a direct effect of alcohol intoxication by a mechanism as yet unknown.
