Two patients with occult renal cell carcinoma had a herald state characterized by the extrarenal signs of fever, "refractory" anemia, and hyperhaptoglobinemia. Nephrectomy initially corrected these abnormalities. The mechanisms of this extrarenal triad and its use in the earlier diagnosis of hypernephroma are discussed.
