Fourteen patients with diabetes and two with borderline glucose tolerance tests who had asymptomatic parotid gland enlargement have been studied. In five the observation of parotid gland enlargement preceded the recognition of diabetes, and in four of these the diagnosis was made only on glucose tolerance test. Sialograms in three patients were normal, and microscopic examination of three parotids revealed fatty infiltration of the gland. The pathogenesis of this sign is not clear. A search for diabetes should be made in all patients in whom enlarged asymptomatic parotid glands are found.
