To the Editor: I read with considerable interest the editorial "Isoproterenol for the Treatment of Shock" (Ann. Intern. Med. 70: 638, 1969). It is clear that the author feels that isoproterenol is indicated when the balance that exists between the beneficial effects of the drug on the circulation and delivery of oxygen exceeds the harmful effects. The latter, it was shown, consists of the production of myocardial ischemia by stimulation of the heart beyond the available oxygen supply.
