Recent pathological and serial ECG studies have suggested that many cases of complete heart block may be due to bilateral block in the bundle branches rather than block in the AV node. The technique of recording the His bundle electrogram with a catheter allows the site of AV block to be differentiated into that occurring proximal or distal to the common bundle. In 16 elderly patients in chronic complete heart block studied at the time of insertion of a permanent pacemaker or at the time of a battery change the block was proximal to the His in only 2 patients
