At The Texas Heart Institute, Houston, we have performed 20 cardiac transplants in 19 patients, including a second allograft in 1 case and a combined cardiopulmonary transplant in another. A two-stage approach to cardiac replacement was implemented using an orthotopic cardiac prosthesis for total cardiac substitution in a 47-year-old man whose heart failed after cardiopulmonary bypass. For 64 hr the four-chambered, pneumatically controlled pump, which was connected to an external control console by tubes passed through the patient's chest wall, effectively supported circulation until a donor heart was secured for transplantation and the prosthesis removed.
