Although pathological Q waves that mimic those of myocardial infarction are often seen in idiopathic hypertrophic subaortic stenosis, their electrogenesis is poorly understood. The Frank vectorcardiographic system was used to analyze septal excitation in 14 patients with proved idiopathic hypertrophic subaortic stenosis whose scalar electrocardiograms showed abnormal Q waves. Initial or septal forces were abnormally displaced superiorly in three cases reflecting deep Q waves in inferior leads of the electrocardiogram. In four patients, the vectorcardiogram displayed large, initial, rightward forces corresponding to prominent Q waves in scalar leads I, aVL and V6.
