To the editor: It has taken me some time to crystallize my dissatisfaction with the article by Dr. Allan Metzger, describing his care of a patient with gonococcal arthritis complicating gonorrheal pharyngitis (Ann Intern Med 73:267-269, 1970). I have no quarrel with Dr. Metzger's careful history and physical examination, nor with his laboratory diagnosis of this patient.
What I am troubled by is the time course between the events of the patient's admission workup, the bacteriologic diagnosis of Neisseria gonorrhoeae infection, and the beginning of specific therapy. If we accept that the culture returned positive 48 hr after admission, the
