The thoracic murmur of splenic arteriovenous fistula in an elderly lady is described. The murmur was continuous, but variations in the ausculatory site and respiratory movements of the spleen caused alterations in intensity of the murmur. At a point slightly cephalad to the maximal bruit, deep inspiration caused the murmur to disappear. At a point caudad to the maximal bruit, deep inspiration caused the murmur to appear.
