There are no generalists in the USSR; all doctors are specialists. There are three main types of primary physician: local district therapists for adults, local district paediatricians, and occupational physicians for workers in industrial plants. In the towns primary physicians work in polyclinics with their own nurses and in association with specialists to whom patients may go directly or by referral. In the large cities there are separate polyclinics for adult therapy, paediatrics, obstetrics and gynaecology, psychiatry, tuberculosis, and so on, in all of which doctors may act as primary physicians.
