The pressure gradient observed in patients with obstructive cardiomyopathy has been attributed to asymetric hypertrophy of the septum. At the level of the ouflow tract, the anterior leaflet of the mitral valve is in close relation to the septum. Mitral regurgitation of variable degree has been associated with this syndrome. In patients requiring surgical relief of the obstruction, division or resection of the abnormal septum has been performed. During the last 2 years five patients with obstructive cardiomyopathy were treated with replacement of the mitral valve; in all five the complete disappearance of the gradient was observed under basal condition,
