A 32-year-old white man with essential hypertension presented with three episodes of acute and severe pulmonary allergic pneumonitis on ingestion of one 50-mg tablet of hydrochlorothiazide. The third episode, noticed during a challenge test, was similar to the previous two episodes. To our knowledge this is the third case of severe allergic pneumonitis secondary to hydrochlorothiazide and the first in which a challenge test was done.
