To the editor: The excellent paper by Miskin, Rosen, and Walfish (1) clearly shows the utility of ultrasound for differentiating cystic from solid thyroid nodules (not an original observation, as they indicated). I agree with their suggestion that combined ultrasound-scintiscan evaluation will increase the accuracy of preoperative diagnosis and thus reduce the number of benign nodules excised unnecessarily. But proof of this hypothesis requires comparative data on the accuracy of the diagnosis of malignancy in thyroid nodules before and after ultrasound availability, data not provided by them.
