Although it is doubtful that gastrointestinal exfoliative cytology can ever attain 100% accuracy, remarkable progress has nevertheless been made with this method in the past 2 decades. The development of the flexible fiberoptic instruments, through which both directed wash and brush cytology can be obtained, has significantly increased the yield in correctly diagnosing gastroesophageal cancer. It is to be hoped that the increased use of flexible fiberoptic colonoscopy will encourage examiners to obtain cytologic washings and brush cytology, since in the United States colonorectal cancer is much more of a problem than gastroesophageal cancer.
