To the editor: I do not think that the case report "Extreme Hypermagnesemia as a Cause of Refractory Hypotension" in the November 1975 issue should have been published. Documentation is sorely lacking. The precise points of occurrence in the clinical course of the repeat X ray that show "free sub-diaphragmatic air" and the "ultimately" performed laparotomy are nowhere mentioned. The discussion is not only inadequate, it is frankly misleading. In a situation where cardiac arrest and a perforated viscus are antecedents, the authors ignore the potential remediable effect of plicating the perforation.
