To the editor: Drs. Fauci, Johnson, and Wolff note in their recent paper "Radiation Therapy of Midline Granuloma" (Ann Intern Med 84:140-147, 1976) the debate and confusion in the literature in distinguishing lethal midline granuloma from Wegener's granulomatosis. They choose to be "splitters" in this regard, stating that the two entities are quite distinct. One of the authors has written elsewhere that primary vasculitis is not a feature of midline granuloma (1). Nevertheless, one of the patients reported in his present series had histopathological findings of small vessel vasculitis and two others had biopsies showing endarteritis. How may one differentiate
