To the editor: At Yale we have treated 30 patients who had stage III or IV diffuse histiocytic lymphoma with a combination of cyclophosphamide, 1.5 g/m2 intravenously on Day 1; vincristine, 2 mg intravenously weekly X 3; followed by methotrexate (MTX), 80 to 120 mg/m2 intravenously weekly X 8 with leucovorin rescue beginning at 24 h; and cytosine arabinoside (cytarabine), 300 mg/m2 intravenously with each MTX dose (COMA). In a few patients the MTX-cytosine arabinoside was given for 3 weeks only. The sequence was repeated three or more times for a total treatment period of 9 months. The median survival
