To the editor: The paper by Drs. Sbarbaro and Bennett in the February issue (Ann Intern Med 86:183-185, 1977) is of great interest to clinicians. Their work-up and development of the problem of hepatotoxicity of aspirin is to be commended. However, I am concerned with their use of aspirin in a "cautious, diagnostic, and therapeutic trial" without any record of the patient's consent. It would seem to me that the danger of using aspirin in this clinical trial warranted permission from the patient and an explanation of potential complications. Although this may have been done, these facts were not mentioned
