To the editor: The French-American-British classification of the acute leukemias and myelodysplastic syndromes recently reviewed in this journal (1) is a step toward order in a previously chaotic taxonomy. Acute myelofibrosis is a rare entity that shares many of the clinical and morphologic features of the myelodysplastic syndrome of refractory anemia with excess blast cells, such as abnormal, small megakaryocytes, hypercellularity, pleomorphic population of immature cells in the bone marrow, and peripheral pancytopenia (2). If acute myelofibrosis is a clinical pathologic entity, then it seems to lie between the myelodysplastic and myeloproliferative syndromes. The following case illustrates this taxonomic difficulty.
