To the editor: The article "Amikacin Serum Concentrations; Prediction of Levels and Dosage Guidelines" (1) is routinely referred to for determining appropriate dosages for aminoglycoside antibiotics. In their study, the authors calculate creatinine clearance by a modification of the method of Cockcroft and Gault (2) that incorporates a standard ideal body weight of 70 kg: C(c) cr male = 140 - ageserum creatinine.
The authors provide a convenient dosing chart for aminoglycosides in Figure 4 of their article. However, the formula for creatinine clearance is erroneously reproduced in the chart as
C(c) cr male = 140 - age/serum creatinine.
