To the editor: The review by Rapp (1) on the adolescent patient fails to emphasize properly the concern and care needed in dealing with the genito-rectal examination, especially with adolescent males. An overly zealous physician neglecting to properly explain the nature and purpose of this standard procedure may induce strong patient resistance, fear, and misunderstanding. For a teenage boy struggling with sexual identity, a genito-rectal examination by a male physician can bring on fears of a homosexual assault and precipitate homosexual panic.
