To the editor: We read with interest the article by Moore, and colleagues (1) on aminoglycoside nephrotoxicity in the March issue, but we differ with their interpretation of the data. They estimated renal function from a calculated creatinine clearance and concluded that "a high initial creatinine clearance is associated with an increased risk of developing nephrotoxicity." On review of the data, it is likely that an initial overestimation of the creatinine clearance was responsible for the development of nephrotoxicity. The higher trough aminoglycoside levels in the patients who developed nephrotoxicity support our interpretation.
