To the editor: The use of leukapheresis in patients with acute myeloblastic leukemia and extreme leukocytosis has been shown to quickly decrease blast counts and improve symptoms in patients developing the hyperleukocytic syndrome, characterized by respiratory insufficiency, central nervous system dysfunction, hemorrhage, and peripheral ischemic events (1-4). We report the case of a patient with acute myeloblastic leukemia and high blast counts who had rapid lowering of blast numbers without the development of the tumor-lysis syndrome after the institution of high-dose cytarabine therapy.
