To the editor: I found the article by Knaus and colleagues (1) fascinating to say the least. I am concerned, however, that no mention was made of the physicians who were involved in the treatment of the patients. The article seems to assume that the quality of the physician involved was equal in all the institutions. I am not certain that this conclusion is justified and wondered how the analysis handled this problem.
