The issues facing clinicians in coronary care units have become increasingly complex. Since these units were first started over 20 years ago, there have been advances in hemodynamic monitoring, circulatory assistance, surgical procedures for acute circumstances, new forms of pharmacologic therapy, and numerous and frequently conflicting studies of interventions to reduce morbidity and mortality due to ischemic heart disease. The authors provide a scholarly analysis of these changes and offer balanced and conservative recommendations for therapy in controversial areas.
