To the editor: The Patient Registry for the Characterization of Primary Pulmonary Hypertension represents an important advance in our understanding of this uncommon disease. The study is prospective in design and includes a large number of patients diagnosed in a standard fashion (1). However, the question still remains as to whether primary pulmonary hypertension can accurately be diagnosed without an open lung biopsy.
The distinction between primary pulmonary hypertension and pulmonary hypertension secondary to occult microemboli can be impossible to make on a clinical basis (2). Identification of patients with embolic disease is important, as effective therapy can be provided
to gain full access to the content and tools.
