To the Editor: We read with interest the article by Zamora-Quezada and colleagues (1) on muscle and skin infarction after free-basing cocaine. We report three cases of myonecrosis that occurred in healthy adult men after they abused cocaine. Muscle pain, tenderness, myoglobinuria, and elevated levels of serum creatine kinase indicated the occurrence of myonecrosis. One patient had the excited delirium syndrome after smoking cocaine; after throwing himself to the ground, he was picked up by police.
