To the Editor: In their editorial on aerosolized pentamidine, Armstrong and Bernard (1) confuse two ethical responsibilities of physicians and thereby render an unethical proscription. The medical community has the responsibility to identify useful therapeutic agents and to encourage their use. The methodology of the double-blind, randomized, controlled clinical trial is well developed, and the information obtained from it may be of the highest value. In the absence of criterion standard information, however, the practicing physician has an ethical responsibility to make the best decision possible using the best available information.
