Breast enlargement or tenderness may be an important, albeit uncommon, sign of the presence of an HCG-secreting germ-cell tumor. However, reversible gynecomastia may also develop in patients with germ-cell tumors several months after completion of cytotoxic chemotherapy. This is related not to disease recurrence but presumably to hormonal influences, since these patients may have elevated gonadotropin levels and an increased estradiol–testosterone ratio (1, 2). A normal serum HCG level in this setting should reassure both patient and physician.
