Prostate cancer is the second leading cause of death due to cancer in men in the United States (1). Although physicians initially used androgen deprivation therapy (ADT) to treat advanced prostate cancer, ADT is now frequently used for patients with nonmetastatic disease. The use of ADT has increased 2- to 4-fold in the past 10 years (2, 3), and up to 45% of white men who select conservative management for prostate cancer receive ADT (4).
