one more consideration
If a bruit was heard in this man, another consideration might be an aortic aneurysm. Since he is above 65 ( I don't recall his smoking history), and has hypertension, the bruit might indicate another reason for getting an ultrasound, not of the renal vessels, but of the aorta.
