Although study results vary widely, a hospitalized patient has about a one in 10 risk of being the subject of a medication error. To reduce this risk, prescribers, pharmacists, nurses, and administrators should be familiar with the contents of this book.

Since 1975, the authors have had anecdotal reports of medication errors published in Hospital Pharmacy. The major portion of the book includes 150 of these errors. The 18 chapters discuss various categories of medication errors with ample reference to reported errors.
