  • The postoperative pneumonia risk index described in this paper identifies patients at risk for postoperative pneumonia and may be useful in guiding perioperative respiratory care.

  • Extended prophylaxis with low-molecular-weight heparin showed consistent effectiveness and safety in the trials included in this systematic review (regardless of study variations in clinical practice and length of hospital stay) for venographic deep venous thrombosis and symptomatic venous thromboembolism. The aggregate findings support the need for extended out-of-hospital prophylaxis in patients undergoing hip arthroplasty surgery.

  • Use of implantable cardioverter defibrillators (ICDs) or amiodarone in patients with past myocardial infarction and severely depressed left ventricular function may provide substantial clinical benefit at an acceptable cost. These results highlight the importance of clinical trials of ICDs in patients with low ejection fractions who have had myocardial infarction.

Brief Communications

  • According to the results of this survey, many physicians would be willing to be involved in the execution of adults despite medical society policies. The medical profession needs to be better informed about the ethical issues involved in physician participation in capital punishment.

  • This article presents an approach to identifying, appraising, and adapting an established curriculum as an alternative to developing a new one, and reviews a published managed care curriculum as an educational case example.

Review

  • Perianal fistulas are a frequent manifestation of Crohn disease and can result in substantial morbidity, including scarring, continual seepage, and fecal incontinence. The authors review the epidemiology, pathogenesis, diagnosis, classification, and medical and surgical treatment options for fistulizing Crohn disease.

Editorials

  • With publication in this issue of Arozullah and colleagues' study, the sleeping giant of risk prediction for postoperative pulmonary complications stirs. This study is a significant step forward in preoperative pulmonary risk assessment and helps us to establish priorities for future research.

  • In the hard-fought effort to keep the white coat clean of the association with the black-hooded executioner, the findings of Farber and colleagues in this issue provide a wake-up call. It is time to eliminate moral confusion by reestablishing the deliberate dissent of the medical profession regarding physician involvement in executions.
