    Animation of a 3-dimensional reconstruction of data set obtained by multiphase postmortem CT angiography from a patient with cirrhosis who died of hemorrhagic shock after a perforation of the superior vena cava by a dialysis catheter (case 3 of Table 2). Soft tissues are faded digitally; thus, only osseous structures, organs with high perfusion (for example, the liver, spleen, and kidneys), and medical devices are visible. The dialysis catheter entering the left jugular vein and perforating the vessel at the junction with the brachiocephalic vein and the superior vena cava is clearly visible. A fixation bias on the diagnosis “septic shock” might explain why the perforation was not recognized on the corresponding antemortem chest radiograph (Figure 3).
