Idiopathic generalized myelofibrosis with myeloid metaplasia is a disease of unknown etiology in which the clinical and morphologic manifestations vary greatly even during the course of an individual case. The resulting confusion has led to an undue number of synonyms, each of which tends to focus on a single aspect of the disease. Many cases exhibit panhyperplasia of all bone marrow elements including fibroblasts, although eventually the more characteristic picture of fibrosis may predominate.
