A 39-year old female acutely developed severe acidosis following ingestion of outdated tetracycline. Studies with the ammonium chloride loading test done 2 months after the acute episode showed a persistent renal tubular defect in hydrogen ion excretion. This tubular disorder appeared to be a primary deficiency in renal tubular cell hyrogen ion production or transport, or both. There was no evidence for defective renal tubular bicarbonate re-absorption, inadequate ammonia formation or insufficiency of phosphate ion excretion to act as a hydrogen ion acceptor.
