Study of the prognosis and natural history of the chronic diseases has long been of consuming interest to many types of workers who approach the subject from varying points of view: on one end of the spectrum are those concerned with the basic biology and fundamental nature of disease processes; in the middle are the practical physicians who are concerned with optimal treatment; and at the other end are those whose concern is highly utilitarian, such as the insurance actuaries who must try to calculate appropriate premium rates based on defined risks.
