Ferrokinetics have been used to differentiate primary, secondary, and relative polycythemias. Since the differences in ferrokinetics might be explained by the consistently depleted iron stores in untreated polycythemia vera in contrast to the normal or increased quantities in uncomplicated secondary polycythemia, the relationship of iron stores to ferrokinetic patterns in polycythemia was studied.
Iron studies were performed on 13 patients with typical primary and secondary polycythemia in whom storage iron had been assessed. If iron stores were depleted, sufficient parenteral iron was given to replete the stores, and the studies were repeated.
