Central venous pressure is used clinically as an index of myocardial function in shock, but the relationship between right and left heart filling pressures in this situation has not been analyzed. Simultaneous measurements of right atrial (RAP) and left ventricular end diastolic (LVEDP) pressures were performed at the bedside of 18 patients with shock (9, acute myocardial infarction; 6, sepsis; 2, pulmonary embolism; 1, hepatic failure).
In acute myocardial infarction LVEDP ranged from 14 to 30 mm Hg (average, 21 mm Hg) and was always at least twice the RAP (range, 3 to 14 mm Hg, average, 8.7 mm Hg).
