Clinicians have long been aware of a bleeding tendency in uremic patients. Recently there have been several reports that the platelets of uremic patients have decreased adhesiveness to glass. We measured platelet aggregation by the decrease in optical density which occurred when platelet-rich citrated plasma was agitated with an aggregating agent. We found decreased adenosine diphosphate-induced platelet aggregation in the platelet-rich plasma of 20 nonuremic men after increasing urea nitrogen concentrations by 100 mg/100 ml (P < 0.01) and 200 mg/100 ml (P < 0.001). Increasing concentrations of creatinine by 10 mg/100 ml and 20 mg/100 ml had no significant effect.
