Profound acute hypocalcemia was associated with a histologically proved infarcted parathyroid adenoma in the patient reported. The hypocalcemia was presumed to be the result of rapid skeletal recalcification after the sudden fall of the circulating parathyroid hormone level accompanying the infarction of the parathyroid adenoma and the inability of the remaining parathyroid glands to secrete adequate hormone. Improvement of the severe skeletal demineralization was documented. This is the fourth reported case in which the course of primary hyperparathyroidism was reversed by spontaneous infarction of a parathyroid adenoma.
