A dehydration test has been developed which distinguishes normal subjects and those with primary polydipsia from patients with diabetes insipidus and allows the identification of partial antidiuretic hormone (ADH) deficiency. The test consists of depriving patients of water until hourly voided urine osmolality (Uosm) reaches a plateau and relating the Uosm at which this occurs to that which follows subsequent ADH injection. In normal subjects and patients with primary polydipsia Uosm at the end of water deprivation is much greater than plasma osmolality (Posm) with no increase after ADH injection. Posm in these subjects is similar
