A teenage girl with recurrent, painful ecchymoses was a difficult diagnostic problem until the factitious nature of her illness became apparent. This type of case must be distinguished from patients with autoerythrocyte sensitization, sensitivity to deoxyribonucleic acid, red cell stroma, and hemoglobin. A diagnosis of factitious purpura was established in this patient by skin tests, direct observation, and the effective use of plaster casts. Factitious purpura is a cause of recurrent, painful ecchymoses.
