The response of myocardial blood flow to right atrial pacing and isoproterenol was correlated, in 75 patients, with the severity of arteriographic coronary disease, quantitated by number and severity of vessels involved (coronary artery index) and the ECG response during treadmill exercise. Right atrial pacing has been shown previously to increase myocardial blood flow from 10% to 58% in normal subjects. Of the 16 patients in this study 9 were unable to increase myocardial blood flow with right atrial pacing (+3 to -27%); 8 had 1 positive and 1 equivocal treadmill exercise (P < 0.001).
